case 7 Physical exam

VITAL SIGNS: BP:  98/ 60, HR: 84, RR; 20, O2 sat 100%, weight: 90lbs , Height: 5’2 ” BMI 15.6

General Appearance: thin woman, in no acute distress, mild bitemporal wasting

HEENT: Normal conjunctiva, no scleral icterus, moist mucous membranes, clear    oropharynx
 

Cardiovascular: No JVD, regular rhythm, no rubs/murmurs/gallops

Respiratory: decreased breath sounds  with rales at the right lung base

Abdominal: soft scaphoid abdomen, well healed vertical midline scar, normal bowel sounds. No ascites, hernias or organomegaly appreciated

Extremities: warm and well perfused extremities, without edema

Neurologic: Alert and oriented X 3 without focal deficits
