What are the next best tests you should order?
Computed tomography (CT) of the abdomen and pelvis with IV contrast
Correct! The next step would be to obtain abdominal imaging (CT imaging if available, or KUB/ upright abdominal X ray) to determine if the patient has a bowel obstruction and ascertain the site of obstruction.
Celiac serologies
Incorrect, try again!
Since the patient has no history of gluten intolerance and is not experiencing chronic diarrhea or bloating, the suspicion for celiac disease is low
Fecal calprotectin
Correct!
Obtaining fecal calprotectin is also a reasonable choice , as this is a non-invasive marker of colonic and small bowel inflammatory activity. Getting a baseline value is a good idea to monitor response to therapy
Serum CRP
Correct! serum CRP is an indicator of inflammatory activity. If the patient is unable to provide a stool sample, serum CRP is also a good choice to monitor response to therapy.
Colonoscopy with biopsies of the colon and small bowel
Incorrect! Try again
The patient is unlikely to be able to tolerate a colonoscopy prep with her current obstructive symptoms, so colonoscopy is not an ideal test at this time.
