History of Present Illness (HPI)

52-year old Caucasian man with medical history significant for hypertension , chronic gastro-esophageal reflux disease (GERD) and Type 2 diabetes presents with several months of progressive dysphagia. Dysphagia is  initially to solids but over the past 2 weeks he has developed dysphagia to liquids too. He reports 20 pound weight loss since his symptoms started .

Meals over the past week consist of sips of soup, protein shakes or Ensure

Current medications include:

  • Hydrochlorothiazide 25 mg, Famotidine 40 mg and Metformin 1000 mg daily
