History of Present Illness (HPI)

  •  A 52-year old Caucasian man with medical history of hypertension, chronic gastro-esophageal reflux disease (GERD) and Type 2 Diabetes presents with several months of progressive dysphagia.
  • Dysphagia was initially to solids, but over the past 2 weeks he has developed dysphagia to liquids as well. He also reports 20 pound weight loss since his symptoms started .
  • Meals over the past week consist of sips of soup, protein shakes or Ensure
  • Current medications include: Hydrochlorothiazide 25 mg, Famotidine 40 mg and Metformin 1000 mg daily
