Case 1 continued

• In the early hours of next morning, the RN goes in to give your patient his morning medications and finds him lethargic, with muscle weakness and shortness of breath.

Vital Signs: 

HR 120, irregular pulse, O2 sats: 78%, RR: 7

• A rapid response is activated, and the patients is started on Oxygen supplementation. As is respiratory distress is getting progressively worse, a decision made to intubate him and transfer to ICU.

• 12 lead EKG is performed and is attached below

• Repeat labs show

Sodium: 140 Meq/ L,  K: 2.8 Meq/ L ,

Phosphorous : 0.8 mg/dl and Magnesium 1.2 mg/dl
